FYDP is designed to meet the unique needs of Transition Age Youth in the Alameda County foster care system and to support these youth as they prepare for and navigate through emancipation. The job title is FYDP Transition Facilitator, who will provide a wide array of services to support foster youth from 15-years-old through their 21st birthday. The Transition Facilitator provides intensive clinical case management, individual, family and group therapy, as well as collateral work with foster parents, social workers, teachers, and other adults in youths’ lives. FYDP services are primarily community-based, meeting youth where they are at and where they feel most comfortable, whether that is in their home, school, or in the community. The Transition Facilitator must have strong clinical and advocacy skills, be flexible, utilize a strengths-based approach, and demonstrated experience working with adolescents in foster care.
